EXPIRED - America's Wellness Challenge - Ends 2/28/2010

This sweepstakes does allow for online entry, but there are some questions you must answer to complete your registration that seem a little nosy to me. If you are nervous about disclosing health-related information online, like I am, you can also enter by mail.

One grand prize winner will receive $15,000 cash.

How do I enter to win?

Or, you can hand print your name, complete address, city, state, zip code, birth date and email address on a 3" x 5" card and mail it to:

You are limited to one entry per person, either via the online or mail-in method. If you choose the mail-in method, your entry must be postmarked by 2/28/10 and received at the above address by 3/10/10. If you choose to enter online, you must do so by 11:59 PM ET on 2/28/10.

To participate in this sweepstakes, you must be a legal resident of the 50 United States and D.C. and be at least 18 years of age at time of entry.
